I, volunteer to serve on the RAIN Community Technology Advisory Council for the community of , California. I agree to serve as a volunteer member of this Council for a term of 6 months 12 months, commencing and concluding . During my term on the Council, I will attend monthly online meetings and quarterly Council meetings.

I understand that my responsibility to the Council is to serve as a liaison between the RAIN Network's Rural Community Network 2000 project and my community.

I understand the primary role of the Community Technology Advisory Council (CTAC) is to provide recommendations regarding the development of internet-based Community Education and Telemedicine services.

As a member of the CTAC I will assist in gathering resources for the creation of a website for my community, and provide comments and recommendations regarding the creation and maintenance of this website. I will also offer input to the Council regarding the online computer technology needs of my community.
